Please download and fill out a Patient Registration Form in advance and then bring it with you when you come to the hospital.

On the day you check into the hospital, please come to reception on the 1st floor to do the admissions application.

Our hospital has paid parking. Please show your parking lot ticket to one of the receptionists when you enter the hospital so that you can get a discount on parking.
*The access road on the south side of the hospital is a school zone from 7:30 to 8:30 in the morning so access to the hospital via this route is prohibited. If you come to our hospital during this time please enter from the north entrance of the hospital.
